After the Move Form Employee Questionnaire
This is a form that should be given to each of your employees after the move. This form gives each employee a chance to review the equipment in their offices and in their departments and confirms that everything is working and that all the accessories (such as plugs & cables) have been received. It also asks the employee to confirm that the corresponding wall jacks are working and don’t need attention from your company’s IT Department.

This form gives you a place to gather up and keep track of everyone’s phone numbers, office extensions, new office numbers, etc., and is intended to be posted in a central location or to be given to each employee or team member as a resource that they can use both during & after the move.
